After we receive your referral, it will be triaged & reviewed. The client will be contacted directly by the Admissions Staff if an appointment is scheduled. To ensure the client’s referral is reviewed, all sections of the form must be completed.  If the referral form is not complete and/or your writing cannot be read, it will be returned to you with an explanation stating why you need to fix it or send us a new referral. All information on the referral form is important, but if it does not apply, then please write N/A in that section.

Referral Forms must be accompanied by enough Release of Information forms, signed by the client, for us to contact the other providers/hospitals involved in the case, otherwise the referral cannot be triaged.

Community Referral

Behavioral Health Home services provide care coordination and education to support addressing mental and physical health concerns. Behavioral Health Home Services include Comprehensive Case Management support, that focuses on addressing the client’s physical and behavioral health needs, and promotes community and home-based recovery. This team approach to treatment involves a care coordinator, peer and family supports, a nurse and engages primary care service providers with the goal of promoting wellness. This is a Maine Care funded service.

The center offers a relaxed atmosphere with support and socialization. Participants are welcomed and treated with respect and dignity. In addition to assistance linking participants to services located in the community based on need such as housing, behavioral health treatment and vocational rehabilitation services, participants have access to peer support, structured group support, and self-help activities. These services can help people overcome physical, economic and social barriers that they normally encounter.

The Beacon House Peer Recovery Center is open every day, Monday through Friday, from 8:30am to 4:30pm. Coffee and canteen service is always available, and lunch is available each Tuesday at noon.

The Beacon House Peer Recovery Center is a member of the Maine Association of Peer Support and Recovery Centers (MAPSRC) and is recognized as an evidence-based model of care through incorporation of community integration. Case Managers help clients increase independence and quality of life by providing the support and motivation needed to deal with mental health issues.

Case Management clients complete a comprehensive assessment and then work with their case manager to identify specific goals. Goals are based on issues that are important to the client, and provide a step by step plan which will reduce barriers that have gotten in the way before. Case Managers then support clients in doing the work to achieve those goals.

OCCR provides county wide emergency outpatient mental health services on a 24/7 basis at agency offices, in the community and in hospitals. OCCR provides crisis intervention, aftercare, and referral services. Call 888-568-1112.

Aftercare
Aftercare services are a maximum of 5 sessions that are focused on resolving the problems identified in the first contact with OCCR staff. Crisis Stabilization Unit
The Jane deFrees Health Center
The Assessment and Stabilization Unit, located in the Jane deFrees Health Center at 150 Congress Street in Rumford, is an inpatient alternative to hospital admission. The person receives a full assessment of their needs with a goal of returning home within 5 to 7 days. The emphasis is on rehabilitation. This is a fully modern ADA facility with fully qualified staff. The crisis stabilization unit provides needs assessment and stabilization, not hospitalization. Call us at 207-364-3030 to learn more.

For many adults diagnosed with major mental illness the issue of growth and recovery is severely limited by the inability to effectively modulate and manage emotions given any perceived “prompting event.” A prompting event is any internal or external stimuli that leads to emotion dysregulation (i.e. phone calls, community integration, mail delivery, peer interactions, flashbacks, anniversaries, natural disasters, or deaths).

The inability to regulate emotions creates a life of hopelessness and builds a life with no meaning for the individual. This will often lead to disruption and very often the dissolution of meaningful relationships either natural or professional. Additionally, this can lead to the perpetuation of the “revolving door phenomena” (the rapid and continual admission and discharge within an acute care facility) often seen within the mental health community.

Often times if the individual had been able to identify the prompting event, recognize the trigger, experience and accept the emotion in the moment, and accept the natural outcome of the situation, they would have been able to successfully manage and regulate their emotions.

The goal of Dialectical Behavior Therapy (DBT) Skills Group is most simply to assist an individual to create a validating environment in which the individual will manage any emotional dysregulation.

Outpatient Therapy offers a planned number of sessions that can be individual, group or family therapy. The focus is on the person developing insight into their life and developing new, more effective ways of thinking, feeling and acting so they can achieve their potential for a well balanced lifestyle.

Andy’s Place, located in Rumford, is a community residence and rehabilitation program for people suffering from chronic and persistent mental illness. Long and short term goals are developed with the resident so they can participate in community living to the extent of their ability, assisted by staff in a caring and supportive manner. Complaints and Grievances Policy

Community Concepts Inc. aims to resolve complaints at the agency level before they escalate to grievances. Clients have the right to file complaints regarding any aspect of the agency’s services. Clients may escalate unresolved complaints to the Department of Health and Human Services (DHHS) or the Office of Substance Abuse and Mental Health Services, as applicable.

Complaint Resolution Process

All complaint resolution steps must be documented as outlined below. If unresolved, a complaint may become a grievance handled by DHHS or the Disability Rights Center. At each stage of the complaint/grievance process, clients have the following rights:

1. Attempt to resolve the concern directly with the involved employee.
2. File a formal written complaint with the Privacy Officer if unresolved.
3. Access assistance from a representative of their choice.
4. Access advocacy from DHHS or the Maine Mental Health System’s Office of Advocacy.
5. Review grievance-related information, excluding confidential details about others.
6. Present evidence or witnesses.
7. Receive findings and recommendations, excluding confidential details about others.
8. File a grievance with DHHS using the available procedures.

Client Complaint Procedure

CCI will investigate all complaints and escalate as needed for a thorough review.

1. Clients should file a Client Complaint Report form for agency processes, procedures, or employee conduct concerns.
2. Clients may receive and complete a copy of the complaint form at their discretion.
3. Employees must notify the appropriate program manager or Privacy Officer of verbal complaints within 24 hours.
4. Completed forms should be submitted immediately to the Privacy Officer.
5. Allegations of serious violations (e.g., abuse, fraud, touching, coercion, threat) require a formal complaint and full investigation and involve the Privacy Officer.
6. Investigations will begin within five business days.
7. Investigation findings and recommendations will be provided to the Director within ten business days.
8. The client will contact within fourteen business days following the complaint.
9. If unresolved, clients may file a grievance with DHHS.
10. All complaint-related actions will be documented on the Client Complaint form.
11. The involvement of legal counsel is at the Director’s discretion.
12. If substantiated, disciplinary actions will be taken against the employee involved.
13. The Client Complaint form, with redacted identifying information, will be placed in the employee’s personnel file.

Client Grievances

Clients may file grievances regarding rights violations or questionable practices and will receive a written response with reasons for the decision. They may request assistance in filing a grievance.

1. Any grievance-related correspondence from DHHS or the Disability Rights Center must be forwarded to the Director.
2. The Director will convene a meeting with relevant employees to address the grievance or respond to requests from DHHS or the Disability Rights Center.
3. Requested records will be provided as guided by the Director.
4. Documentation of resolution efforts will be sent to DHHS or the Disability Rights Center.
5. Employees involved in substantiated grievances may face performance development or disciplinary actions.
6. The Director will maintain a record of client files related to grievances.

Right to Information on Licensing Deficiencies

1. Clients will be notified of any deficiencies identified during licensing surveys or investigations.
2. Clients and their legal representatives at the time of intake will be informed that survey results are public information and are available upon request.
3. Clients and their legal representatives will be notified of action against the organization’s license within fifteen business days. This includes decisions like a Directed Plan of Corrections, conditional license issuance, non-renewal of the license, appointment of a receiver, or sanctions such as fines.
